With expertise in each of these areas, SALMON is able to uniquely provide a continuum of care to accommodate its clients’ ever-changing needs. This means, no matter what stage of life or health a person is in, SALMON caretakers can easily transition him or her to the best service and location for his or her needs, without wait or worry.
These services within the continuum have proven themselves time and time again through multiple partnerships, awards and ratings from esteemed organizations and members of the local community. The SALMON organization has the highest multisite average rating in the state for nursing homes as rated by the Centers for Medicare and Medicaid Services (CMS), and has received deficiency free surveys from the department of Public Health and Commitment to Quality Awards from the American Health Care Association (AHCA) and National Center for Assisted Living (NCAL). Several SALMON communities have also received praise from the community, winning several reader polls through local newspapers and other media outlets.
